Work Permits
The MMSD must abide by rules that have been created by the State of Wisconsin Department of Workforce Development pertaining to students under 16 getting a job.
We are able to process a request for a Work Permit at either site. For Eastside, request in Room 301 and for the Westside, please see the Front Desk. We are not able to process Work Permit requests while classes are in session, please plan to request before school, during lunch or after school. There is no need to seek a Work Permit at your school of residence.
In order to receive a Work Permit, you will need to bring to the Permit Office the following:

Your Original Birth Certificate (not a copy)*

Your Original Social Security Card (not a copy)*

Permission from a guardian

Information from your employer stating you have been offered a job, the duties and hours of work.

$10.00 application fee, which is refunded by your employer on the first paycheck.
*Please note: you may also bring a previous Work Permit instead of your birth certificate and Social Security card.
What If I don’t have a Birth Certificate?

A birth certificate is an essential piece of identification and is necessary for individuals to prove their age.

We may also accept a valid operator’s license or an identification card issued by the Wisconsin Department of Transportation.

We may also accept, at the officer’s discretion, a verified baptismal certificate or a valid passport.

You can receive a new birth certificate if you were born in Wisconsin at the Vital Records Office at 1 West Wilson Street in Madison. The cost is $20
What if I don’t have a Social Security Card?

You will need to visit a Social Security Office and request a replacement Social Security Card.

The SSA office can provide you with a letter stating that you have requested a replacement that a Permit Officer can accept for a Work Permit.
